Flu Shots

Brookhill Industries and Day Hab is teaming up with the Putnam County Health Department to provide flu shots to our individuals and staff if they are interested and have not gotten their dose for the year yet. They will be on site on Thursday, October 14, 2021 starting at 12:30. We will have forms in the office to hand out to anyone interested later this week. Please be sure to turn in your insurance card along with Medicaid / Medicare card for Heather to make a copy to send it with your forms. If you have any questions, call the office at 419-876-3932.
